1. If your students are in the same age cohort, please select your preference. 2. Will there be any other adults (besides the two listed) that attend Trellis with your student? Please list their name, relation to student, and number. 3. Would any of the adults in the family like a Trellis shirt? We tie-dye them during the welcome back party. You may select more than one size if multiple shirts are requested.

4. Please list an emergency Contact- Name and Number. Cannot be the caregiver who will be primarly on site with the students. 5. We work with many materials, including food. This co-op requires caregiver buy-in and involvement. You agree to volunteer your time every week to assist at table, with a group, or as lead teacher, clean up, and 4 or more weeks a year you agree to arrive early and help set up.

10. Trellis' fees are as follows:

Registration Fee: $15/family (pays for website for the year)

T-Shirts - $8/shirt

Monthly Dues - $10 per first child, + $5 per additional child 4 years old as of Sept 1st or older, capped at $20/family.

11. If I no-call no show 3 weeks in a row OR 4 times during the 2026-2027 year my family will be unenrolled from Trellis, and I will not be eligible for a refund.

12. Trellis is an inclusive co-op. Once your registration is received you will be emailed our handbook. You must sign that you agree to abide by the guidelines laid out in the handbook in order to be accepted into the co-op.
